Root Cause Analysis (Part 2)

The Why’s is an interrogative technique used to explore the cause-and-effect relationships underlying a particular problem. Here is an example.

The boss wants to know why a customer wont pay for his new business flyers? Using the Whys Analysis we can answer his question.

1) Why – Does the client not want to pay us?

He says they arrived to late for his meeting and now he doesn’t need them.

2) Why – Did it take you a long time to send the flyers?

Difficulties in procuring the correct materials.

3) Why –  How did the purchasing team not order the paper in a timely fashion. Nobody bothered to check inventory in stockroom, they got word too late.
